Application Process for New Client Hippotherapy

Initial Assessment:
Procedure:
Upon receipt of ALL completed forms listed below, our intake
therapist will review the applicant’s package to determine if our
hippotherapy or therapeutic riding program would benefit the applicant. An
assessment appointment (necessary for ALL applicants) will be scheduled only
if there is a mutually agreed upon weekly session time available. If an opening is not available, the
applicant will be placed on a wait list. Required Assessment Forms:Please complete
the following forms and obtain the required signatures (including
physician). To print each form,
